If you did not renew your license by December 31st of the year the license expired, your license will be placed in a suspended status.
To reinstate your license, you will need to submit the Reinstatement Application(PDF, 513KB) and all required documents for review by the Licensing Supervisor. Reinstatement, renewal, and late fees will apply.
Cancelled Status
Any individual who fails to renew his/her license prior to December 31st of the year following its expiration, shall thereby automatically have a license that is null and void (cancelled).

Second Entity
The Second Entity Application(PDF, 311KB) is to be completed by qualifiers requesting to qualify a second entity within the same trade or requesting a change to an existing second entity qualification. The applicant and presidents, partners, and/or owners of all companies involved must sign the application.
All applicants must appear before the Contractor Licensing Board for final approval. The Board, at their sole discretion, may require additional information in order that the welfare of the consumer is protected at all times.
An application fee is due at the time of submittal of the Second Entity Application and is non-refundable or non-transferable regardless of the outcome for the request of an additional entity. If approved by the Contractor Licensing Board, the initial license fee for the Second Entity will also apply.
Please submit applications through the GMD Public portal, email to ContractorsLicensing@collier.gov or in person.
