Registration and Submittal Requirements

What is a Private Provider?

Florida Statutes define Private Provider under 553.791 Alternative plans review and inspection:

(i) “Private provider” means a person licensed as a building code administrator under part XII of chapter 468, as an engineer under chapter 471, or as an architect under chapter 481. For purposes of performing inspections under this section for additions and alterations that are limited to 1,000 square feet or less to residential buildings, the term “private provider” also includes a person who holds a standard certificate under part XII of Chapter 468. 

Requirements

Florida Statute 553.791(15)(b) authorizes the local government adopt a system of registration:

All required document forms, provided below, shall be used. No substitutions will be accepted, with the exception of the following, which may be provided on company letterhead:

  1. Letter of Acceptance by Private Provider stating services provided to fee owner
  2. Resumes of Private Provider and Duly Authorized Representative(s)

 

Private Provider Registration

Collier County requires Private Providers complete a one-time Registration with Contractor Licensing before commencing work. Private Provider is responsible for keeping registration records current.

  • Registration can be submitted by email ContractorsLicensing@collier.gov, by fax (239) 252-2459, or in person at 2800 N Horseshoe Dr., Naples, FL 34104.
  • Please do not resubmit Registration documents with Project Submittal Packets.

Registration requirements:

  1. Private Provider Registration form(PDF, 403KB).
  2. Private Provider Resume and copies of all Florida DBPR licenses under F.S. 471 as a Professional Engineer, F.S. 481 as an Architect, or F.S.468 as a Standard Building Code Administrator and Standard Inspector for inspections only on residential additions or alterations (of 1000 square feet or less), F.S. 553.791(i).
  3. All Duly Authorized Representatives’ Resumes & Employment Affidavits, signed & notarized. Also, copies of all Florida DBPR licenses of Standard Plan Examiners & Standard Inspectors that are performing inspections or plan review as authorized representatives.
  4. Private Provider’s Certificate of Insurance for general liability & professional insurance meeting State of Florida requirements with Collier County listed as the certificate holder, including 5 years of tail coverage for claims made on policies, per F.S. 553.791(16).
  5. Workmen's Compensation Insurance showing Collier County Contractor Licensing Board as the Certificate Holder and/or a copy of Workers' Comp Exemption filed with the State. NOTE: Qualifier(s)/License Holder(s) are required to be listed on the policy as “Included” or “Excluded”.
  6. Copy of Driver's License for Private Provider and Duly Authorized Employees.

 

Permit Submittal

Documents required with building permit application submittal:

  1. Notice to Building Official(PDF, 357KB), signed, sealed & notarized (2 pages).
  2. List of Building Inspections(PDF, 371KB), for all trades, if not using County selected inspections. Required for plan review.
  3. Plan Compliance Affidavit(PDF, 258KB), signed and notarized. Required for plan review.
  4. Spot Survey Affidavit(PDF, 257KB), signed & notarized, if spot survey required.
  5. Private Provider must submit signed and sealed plans when required by the Florida Building Code per Collier County Building Blocks/Bulletins for the type of construction or project being built.

Note: If the private provider or duly authorized representatives are not licensed in ALL trades for plan review and inspections, they will be put on RESTRICTED status and will be required to upload all registration documents, including insurance & duly authorized representative documents, in addition to the NTBO, with every permit submission.

 

Inspection Reports and Certificate of Compliance

Inspection Process:

Private Provider Inspection Reports(PDF, 205KB) must be filled out completely and signed by the private provider or duly authorized representative.

In accordance with F.S. 553.791(9), private providers performing required inspections shall provide notice to the local building official, such notice will be given by the private provider scheduling the inspection date via the GMCDD portal or phone scheduling system at least the day before the inspection is to be performed.

In accordance with F.S. 553.791(11), the private provider shall report the results to the building official. Inspection reports will now be uploaded to the Private Provider Inspection condition on the GMCDD portal within 2 business days of performed inspection. Reports must be completely filled out and signed either with a written or electronic signature.

a. Any required inspection affidavits are required to be uploaded with inspection report: re-roof, solar, etc.

b. If partial inspections were performed, they should be provided at a minimum of one per floor. Do not provide a PASS report until all the inspections of that type are complete.

c. If inspector notes required revision, a CO hold may be placed on the permit until the revision has been approved. Revisions shall be provided when deemed required, not at the completion of the project.

Document required after all inspections and before Certificate of Occupancy:

Private Provider Certificate of Compliance(PDF, 310KB) (request for certificate of occupancy) must be filled out completely, digitally signed or notarized. All required inspections must be completed prior to submitting the COC.

In accordance with F.S. 553.791(12), upon completion of all required inspections, the private provider shall prepare and upload a certificate of compliance. The provided Certificate of Compliance will no longer need to be a complete package.  Only the Collier County Certificate of Compliance form, digitally signed or notarized, will be required to be submitted to the Private Provider COC on the GMCDD portal. You do not need to resubmit all of the reports. Do not upload your COC until all inspections have been recorded.
