Emergency Home Energy Assistance for the Elderly Program (EHEAP)

The Emergency Home Energy Assistance for the Elderly Program (EHEAP) assists low-income households, with at least one person age 60 and older, when the households are experiencing a home energy emergency. EHEAP is a 100% federally funded home energy assistance program. Eligible seniors may receive up to $2,000 in utility bill assistance per year. For assistance, call (239) 252-2273, or dial 311. To view the application, visit:

Medication

This service provides assistance with medications prescribed by a physician to individuals at 150% poverty level or below, and assistance for elders and individuals considered disabled on Social Security who are 150% poverty level and below.

Temporary Shelter Assistance

This service provides short-term rental assistance for individuals with no income, who are temporarily disabled, where eviction is imminent and have been denied services from other Collier County agencies.

Health Care Responsibility Act (HCRA)

Mandated by Florida statute, this service provides for payment to hospitals where indigent residents of Collier County who are treated at another hospital in the state. The treating hospital must apply for assistance to determine eligibility.

Indigent Cremation Assistance

Collier County Human Services provides cremations for homeless and low-income citizens who cannot afford for the final disposition of a relative. If you have a family member, who has recently passed and cannot afford cremation, you may qualify to have your relative cremated at no cost. The Indigent Cremation program does not provide for a funeral, memorial viewing, visitation, or any other services.

Qualifications for the program include:

  • The person must have died in Collier County
  • The deceased person must not have any form of life insurance.
  • The household income must not exceed 100% of the federal poverty guidelines, and household assets must not exceed the maximum amount.

Documentation requirements:

  • Social security card or other document verifying deceased person's social security number.
  • Photo ID of the deceased person.
  • Photo ID of applicant.
  • Proof of all of household income. All household members the age of 18 and older will have to income qualify in order to receive assistance.

Indigent: A deceased is considered indigent when, at the time of death, the individual meets 100 percent of the federal poverty level recognized by the Federal Income Guidelines produced by the US Dept. of Health & Human Services.

How to Apply?

To apply for services you may contact a case manager to set up an appointment by calling (239) 252-CARE (2273).

An application form will be completed at the time of your interview. You will need to bring the following information when you come for your appointment:

  • Proof of income, such as a pay stub, for the (4) weeks prior to your application, or a letter from Social Security or VA stating monthly benefits
  • A Social Security card and identification for all household members
  • Two residency documents proving that you live in Collier County
  • Proof of assets, such as bank or credit union statements
  • Any insurance policies you own
  • Proof of legal status in the United States
  • Valid Alien registration card
  • Letter from Medicaid stating denial of Medicaid services

In order to receive support, the resident must not be eligible for other assistance programs. Case managers will assist you in determining eligibility for other programs.

In order to be considered a Collier County resident, a person must have living arrangements in the county that the person considers to be his or her home. It is the responsibility of the person applying for assistance or the guardian of such person to provide documentation of Collier County residency.

An item from the following list must be provided as documentation of residency.

  • Receipt of mortgage or rental payment or letter from a landlord. (Motel receipt is not acceptable.)
  • Valid drivers license with Collier County address.
  • Proof of home ownership within Collier County.
  • Homestead exemption document.
  • Public utility bill in name of applicant or spouse with Collier County address.
  • Collier County voter registration card.
  • Proof of enrollment of applicant's children in school in Collier County.
  • Residency documented through a social service agency's or another county department's case record.
  • Cancelled mail from a federal, state, or county agency addressed to the applicant or spouse at a local address.
  • Written verification from a community agency.
  • Verification from Post Office of mail received at an address in Collier County by applicant or spouse (P.O. Box is not acceptable).
  • Declaration of Domicile recorded with the Clerk of the Circuit Court of Collier County.
